is matched carefully to participant needs, compatibility, and support requirements.

Step-by-step journey
1. Initial enquiry
- Label: Support Coordinators, families, or participants contact AAC Care to discuss SIL
needs. - Outcome: We gather key information about goals, risks, and preferred locations.
2. Suitability & matching
- Label: We review NDIS plans, support needs, and compatibility with existing or
upcoming homes. - Outcome: Clear recommendation on suitable AAC Care SIL options.
3. Meet & greet / home visit
- Label: Participants visit the home, meet staff, and discuss routines and expectations.
- Outcome: Participant and family can make an informed decision.
4. Service agreement & onboarding
- Label: We finalise the SIL Service Agreement, risk plans, and support schedule.
- Outcome: Clear roles, responsibilities, and communication pathways.
5. Move-in & ongoing support
- Label: Participant transitions into the home with structured support and regular
reviews. - Outcome: Stable living environment with ongoing monitoring and improvement.

- NDIS participant with approved plan
- Assessed need for regular or overnight support
- Goal to live in a supported home environment
Using SIL Funding
- SIL line items: SIL is usually funded under Core Supports for participants who require ongoing assistance.
- Working with Support Coordinators: We collaborate to ensure the SIL quote, roster of care, and documentation align with NDIS requirements.
- Quotes & documentation: AAC Care can provide detailed SIL quotes, rosters, and supporting
information for plan reviews and approvals.
